- Transporter name, address, and EPA ID number
- Designated facility name, address, and permit number
- Description of waste: type, container type/number, quantity
- Signatures at origin, transport, and disposal
- Date of each transfer point

Certificate of Destruction

What is a medical waste manifest?
A medical waste manifest (also called a tracking document or chain-of-custody form) is a document that accompanies regulated medical waste from the point of generation to the treatment/disposal facility. It provides a complete record of who generated the waste, who transported it, and how it was treated.
Is a manifest required for all medical waste shipments?
Requirements vary by state. For RCRA hazardous pharmaceutical waste, the EPA Uniform Hazardous Waste Manifest (EPA Form 8700-22) is always required. For general regulated medical waste, many states require a tracking document or manifest — check your state’s specific rules.
How long must manifests be retained?
RCRA hazardous waste manifests must be retained for at least 3 years. Many states require medical waste manifests to be retained for 2–5 years. Best practice is retaining all waste documentation for a minimum of 5 years.
What information must appear on a medical waste manifest?
A complete manifest includes: generator name, address, and EPA ID; transporter name and EPA ID; disposal facility name and EPA permit number; waste description (type, container type, quantity); signatures at each transfer point; and date of each transfer.
What happens if a manifest is not returned after disposal?
If a generator does not receive a signed copy of the manifest back from the disposal facility within 45 days (RCRA) or the state-specified period, an Exception Report must be filed with the EPA or state regulatory agency.
